KNOWN ISSUE WITH MOBILE DEVICES

When accessing the application, we are aware that small screen devices (mobile phones and some tablets) may move backward or forward a page during horizontal scrolling. Currently, we are unable to estimate a resolution. to access the hidden columns on the application, a temporary solution is using a desktop, laptop, or wider screen tablets. Google made changes to the Google Forms application which resulted in this issue. In Spring 2022, we will launch a new application process.
